Forms for fleet customers

Customers can access forms for the following: to request customer support, for banking issues or changes, for account issues or changes, for account disputes, and for TChek account issues or changes.

Dispute forms

Use the forms below for account disputes such as transaction or merchant invoice disputes. Please note: for all scenarios in which you believe fraud has occurred, we recommend you always cancel your card and request a new one. You can do so through the online portal or by calling our customer service team. All WEX Fleet account disputes for fraudulent transactions must be submitted through the online portal.

EFS and Fleet One customers

Use this form to submit a claim to dispute a transaction you believe to be fraud (including lost/stolen cards).
